Who this applies to. This Notice applies only to California residents and supplements our general Privacy Policy. If you are not a California resident, this Notice does not apply to you.
HIPAA note. Information that qualifies as Protected Health Information (PHI) under the Health Insurance Portability and Accountability Act (HIPAA) is generally exempt from the CCPA/CPRA and is instead governed by our HIPAA Notice of Privacy Practices. This California Privacy Notice applies only to non-PHI personal information that TeloLife collects about you (for example, information collected from our marketing website, account administration, and commercial transactions).
In the twelve (12) months preceding the effective date of this Notice, TeloLife has collected the following categories of personal information from California residents, as those categories are defined in Cal. Civ. Code § 1798.140:

Clinical and medical information you submit through our intake form or patient portal is Protected Health Information (PHI) under HIPAA, is collected and processed by our affiliated clinical provider group, and is governed by our HIPAA Notice of Privacy Practices — not by this California Privacy Notice.

We will not use or disclose personal information we collect for purposes that are materially different from those described above without providing you notice and, where required by law, obtaining your consent.
In the twelve (12) months preceding the effective date of this Notice, we have disclosed personal information to the following categories of third parties for a business purpose:
TeloLife does not sell personal information as the term "sell" is defined in the CCPA/CPRA. TeloLife also does not "share" personal information for cross-context behavioral advertising as that term is defined under the CPRA.
We have not sold or shared the personal information of California residents in the twelve (12) months preceding the effective date of this Notice. We have not sold or shared the personal information of any consumer under the age of 16 during that period.
We retain each category of personal information only for as long as reasonably necessary to fulfill the purposes for which it was collected, to comply with our legal and regulatory obligations, to resolve disputes, and to enforce our agreements. Specific retention periods vary based on category and context. In general:
As a California resident, you have the following rights with respect to your personal information, subject to certain exceptions:
You have the right to request that we disclose the categories and specific pieces of personal information we have collected about you, the categories of sources, the business or commercial purposes for collection, and the categories of third parties with whom we have shared that information.
You have the right to request that we delete personal information we have collected from you, subject to certain exceptions (such as information needed to complete a transaction, comply with a legal obligation, or exercise free speech).
You have the right to request that we correct inaccurate personal information we maintain about you.
You have the right to opt out of the sale or sharing of your personal information. As noted in Section 5, TeloLife does not sell or share personal information, so this right is preserved for you even though no opt-out is currently necessary.
You have the right to limit the use and disclosure of sensitive personal information to purposes necessary to provide the services you requested. See Section 8 for details.
You have the right not to be discriminated against for exercising any of your California privacy rights. See Section 12.
Under the CPRA, "sensitive personal information" is a defined category that includes account credentials among other items. TeloLife collects account credentials (your username and password) for the sole purpose of authenticating your access to your account and providing the services you have requested.
We do not use your account credentials for any purpose beyond authentication and security, and we do not share them with third parties for any purpose that would trigger the right to limit. As such, in the ordinary course, the right to limit use of sensitive personal information does not have additional operational impact for TeloLife users. You may still exercise the right at any time by contacting us.
To exercise any of the rights described in Section 7, please submit your request by one of the following methods:
Please include in your request: your full name, email address associated with your TeloLife account (if any), the specific right you wish to exercise, and sufficient details for us to locate your information. We will acknowledge your request within ten (10) business days and respond substantively within forty-five (45) calendar days, subject to a one-time forty-five (45) day extension as permitted by law.
Before we fulfill a request to know, delete, or correct your personal information, we are required to verify your identity. Our verification process is reasonably designed to match the sensitivity of the information involved. In general, we will ask you to confirm details associated with your account, such as your name, email address on file, and recent account activity. For more sensitive requests, we may request additional verification.
If we cannot verify your identity to the degree of certainty required by law, we will inform you and will not fulfill the request. We will not use information you provide for verification for any purpose other than verification itself.
You may designate an authorized agent to submit a California privacy request on your behalf. To do so, your agent must provide us with (a) signed, written proof of your authorization (such as a power of attorney or a signed permission letter), and (b) verification of your own identity through the process described in Section 10. We reserve the right to require you to verify your identity directly with us even when acting through an agent.
TeloLife will not discriminate against you for exercising any of your California privacy rights. Specifically, we will not:
We may, however, charge a different price or rate, or provide a different level or quality of service, if the difference is reasonably related to the value provided to us by your personal information — in which case we will provide you with notice and your consent as required by law.
TeloLife's services are intended for adults age 18 and older. We do not knowingly collect or sell personal information from consumers under the age of 16. If we become aware that we have collected personal information from a minor without the required consent, we will delete that information promptly. If you believe we have inadvertently collected information from a minor, please contact privacy@telolife.com.
Some browsers and browser extensions transmit a Global Privacy Control ("GPC") signal that communicates your preference to opt out of the sale or sharing of your personal information. Because TeloLife does not sell or share personal information (see Section 5), there is no ongoing opt-out to honor. We will, however, recognize a GPC signal as a valid opt-out request should our practices change in the future, in accordance with applicable law.
TeloLife does not currently operate any program that qualifies as a "financial incentive" under the CCPA/CPRA (that is, a program that offers a financial incentive or price or service difference in exchange for the collection, retention, or sale of personal information). If we introduce such a program, we will update this Notice and provide the required disclosures before enrolling you.
Businesses subject to the CCPA/CPRA that process the personal information of ten million (10,000,000) or more California residents in a calendar year are required to compile and disclose annual metrics on consumer requests received. TeloLife does not currently meet that threshold. If we do in the future, we will publish the required metrics in this Notice.
We may update this California Privacy Notice from time to time to reflect changes in our practices or applicable law. When we make material changes, we will update the "Last Updated" date at the top of this Notice and, where required, notify you by email or through our services. Your continued use of our services after an update constitutes your acknowledgment of the revised Notice.
