You have the right to inspect all information obtained in the course of a telehealth interaction and may receive copies for a reasonable fee.
A variety of alternative methods of weight loss management may be available, and you may pursue those at any time.
It is in your best interest to inform your primary care provider and all other healthcare providers of your participation in the TeloLife program.
No specific results can be guaranteed or assured. Individual outcomes vary.
Your continued use of TeloLife services constitutes your informed consent to the use of telehealth in the course of your diagnosis, treatment, and ongoing care management.
HIPAA Consent & Notice of Privacy Practices
The Health Insurance Portability and Accountability Act (HIPAA) provides federal protections for your Protected Health Information (PHI). TeloLife is a HIPAA-covered entity. How We Handle Your Information
Patient information is kept confidential except as necessary to provide services or handle administrative matters related to your care, including sharing with licensed clinicians, licensed pharmacies, and diagnostic laboratories involved in your treatment.
Your confidential health information will not be used for marketing, advertising, or the promotion of any products, goods, or services.
TeloLife utilizes vendors and service partners in conducting business. Any such vendor with access to PHI is contractually required to abide by HIPAA confidentiality requirements through a Business Associate Agreement.
We may remind you of appointments or provide care-related communications by telephone, email, or other means convenient to your care.
Government agencies or authorized payers may inspect records including PHI in the normal performance of their regulatory duties.
Your HIPAA Rights
You have the right to request restrictions on the use of your PHI, though we are not obligated to alter internal policies in all cases.
You have the right to access, amend, and receive an accounting of disclosures of your PHI.
You have the right to file a complaint with the HHS Office for Civil Rights if you believe your privacy rights have been violated, without fear of retaliation.

Financial Consent & Billing Authorization
I understand and accept the following financial terms in order for TeloLife to render services:
A valid payment method is required on file to maintain an active subscription. Any remaining balances for services rendered shall be paid in full.
I authorize TeloLife to submit on my behalf and release any medical records or information necessary to process my consultation order.
I authorize TeloLife to charge my payment method for orders placed, goods received, and services rendered not fully covered by third-party vouchers or credits.
All plans are auto-renewing. I consent to being automatically charged for any active plan unless I explicitly request cancellation before my payment is processed through my patient portal.
I certify that I am an authorized user of the payment method provided, and I understand the refund policy as described in our Terms of Service.
Your continued use of TeloLife services constitutes your understanding and acceptance of these financial terms.
Shipping Authorization
All prescription medications are dispensed according to applicable state and federal law, with the approval of the licensed pharmacist in charge and in compliance with all relevant state and federal medical board and pharmacy board requirements.
By authorizing shipment, you agree to hold TeloLife harmless for any delays or errors that occur during the shipping process and are outside of TeloLife's direct control.
Medication is considered dispensed and the order completed when it is signed out for shipping by the licensed pharmacy — not upon delivery to your address.
You authorize TeloLife and its licensed pharmacy partners to ship medication to the address provided in your intake form or any updated address you provide.
All medications are for your personal use only. Redistribution, resale, or transfer to any other individual is strictly prohibited.
Your continued use of TeloLife services and placement of medication orders constitutes your acceptance of these shipping terms and authorization for delivery to your provided address.
Licensing & Accreditation
How TeloLife is structured. TeloLife is a technology and telehealth platform.
We do not practise medicine and we do not dispense medication. Clinical care is provided solely by
independent, licensed physician groups, and all prescriptions are reviewed and issued by clinicians
licensed in the state in which the patient is located. Medications are dispensed only by licensed
pharmacies, in accordance with applicable state and federal law and with the approval of the
licensed pharmacist in charge.
Accreditation. TeloLife is LegitScript Certified. LegitScript
certification is an independent review of legal and regulatory compliance for healthcare websites
and telehealth services. Our certification can be verified directly with LegitScript at
legitscript.com — we do not host the record ourselves, so that it
cannot be altered by us.
Where we operate. TeloLife serves patients in the
United States only. We do not market to, accept patients from, or ship medication
to any other country. Services are available only in states where our partner physician groups and
pharmacies are licensed to operate.
Prescription-only medication. The GLP-1 medications available through TeloLife
are prescription-only. No medication is supplied without a clinical consultation and a prescription
issued by a licensed clinician. Eligibility is determined by that clinician, not by TeloLife and not
by the completion of any questionnaire.
Licensure records. State pharmacy and dispensing licences are held by the
licensed pharmacies and physician groups that provide care and fulfilment. Copies of the relevant

TeloLife is a technology platform. Clinical care is provided solely by independent, licensed physician groups that adhere to rigorous medical protocols designed for patient safety. Pharmacy Partners: TeloLife partners with multiple USA-licensed, state-certified compounding pharmacies to bring the highest quality product and overall experience to our patients. Our team maintains ongoing relationships with pharmacy partners to address product availability, shipping timelines, and medication quality reporting.
